About The Position

PACE, a member of Empath Health, is seeking a compassionate and detail-oriented full-time Physical Therapist for its interdisciplinary PACE team in Bradenton, Florida. The role helps older adults age in place with dignity by delivering personalized therapy in participants’ homes, facilities, and the Day Center. The Program of All-Inclusive Care for the Elderly (PACE) helps older adults remain independent and safe in their own homes. Care is relationship-based and preventive, with therapists collaborating with physicians, nurses, social workers, and other clinicians to support participants’ independence, mobility, and holistic well-being. The therapist supports restorative, maintenance, and rehabilitation goals in this care model. Requirements

Bachelor’s degree in Physical Therapy.
Active Florida physical therapist license or eligibility for a Florida license.
At least 1 year of experience working with geriatric populations.
Current CPR certification.
Strong communication, time management, and documentation skills.

Nice To Haves

Master’s or doctorate in Physical Therapy preferred.

Responsibilities

Deliver skilled physical therapy interventions in participants’ homes, the Day Center, and facilities.
Collaborate with the interdisciplinary care team to create personalized therapy plans.
Track participant progress and manage documentation.
Coordinate the use of durable medical equipment (DME).
Support restorative, maintenance, and rehabilitation goals in a proactive care model.
