About The Position

Human Touch Home Health is seeking a compassionate and motivated Physical Therapist (PT) to join our growing team serving patients throughout Fredericksburg, Stafford, Spotsylvania, King George, Caroline County, and surrounding communities. As a Home Health Physical Therapist, you'll help patients regain strength, improve mobility, restore balance, and rebuild confidence through personalized, one-on-one care delivered in the comfort of their own homes. You'll have the time and flexibility to create individualized treatment plans while building meaningful relationships that lead to better patient outcomes.

Requirements

Active Virginia Physical Therapist License Required
Valid Driver's License and Reliable Transportation
Strong Clinical Assessment, Communication, and Documentation Skills

Home Health Experience Preferred but Not Required
OASIS Experience Preferred
Experience with WellSky Home Health is a plus
Passion for Delivering Exceptional Patient-Centered Care

Responsibilities

Complete physical therapy evaluations, Start of Care (SOC), reassessments, and discharge visits
Develop individualized plans of care based on patient goals and clinical needs
Provide skilled physical therapy services in patients' homes
Educate patients and caregivers on therapeutic exercises, mobility training, balance, fall prevention, and home safety
Monitor patient progress and modify treatment plans as appropriate
Collaborate with physicians and the interdisciplinary care team
Complete timely and accurate clinical documentation using WellSky
