About The Position

Human Touch Home Health is seeking a compassionate and motivated Physical Therapist (PT) to join our growing team serving patients throughout Blacksburg, Christiansburg, Radford, Pulaski, Floyd, Giles County, Montgomery County, and surrounding New River Valley communities. As a Home Health Physical Therapist, you will provide individualized care in patients' homes, building meaningful relationships, creating personalized treatment plans, and helping patients regain strength, improve mobility, and restore confidence. The role emphasizes providing exceptional care with sufficient time and autonomy, avoiding rushed appointments, crowded clinics, double-booked schedules, and excessive after-hours paperwork. Requirements

Active Virginia Physical Therapist License Required
Valid Driver's License and Reliable Transportation
Strong Clinical Assessment and Documentation Skills
Excellent Communication and Interpersonal Skills

Responsibilities

Complete evaluations, reassessments, and discharge visits
Develop individualized plans of care based on patient goals and clinical needs
Provide skilled physical therapy services in patients' homes
Educate patients and caregivers on therapeutic exercises, mobility training, fall prevention, and home safety
Monitor patient progress and adjust treatment plans as needed
Collaborate with physicians and the interdisciplinary care team
Complete timely and accurate clinical documentation using WellSky
