About The Position

Compassionate Home Health LTD is a locally owned and clinician-led home health agency proudly serving patients throughout the Greater Sacramento Area. Our team is built on compassionate care, clinical excellence, and strong support for both patients and staff. We are currently seeking a motivated and dedicated Physical Therapist (PT) – PRN to join our growing team. In this role, you will provide skilled in-home physical therapy services focused on restoring mobility, improving functional independence, reducing pain, and enhancing overall quality of life. We are looking for clinicians who value patient-centered care and thrive in an autonomous home health environment.

Requirements

  • Graduate of an accredited Physical Therapy program
  • Current and unrestricted California Physical Therapy license
  • Current CPR certification
  • Reliable transportation, valid driver’s license, and auto insurance required

Nice To Haves

  • Minimum of 1 year of physical therapy experience preferred
  • Home health experience preferred, but not required
  • Strong clinical assessment and patient care skills
  • Excellent communication, organization, and time-management abilities
  • Ability to work independently and manage a flexible schedule

Responsibilities

  • Evaluate patients and develop individualized treatment plans based on physician orders and patient needs
  • Establish measurable patient goals and implement evidence-based physical therapy interventions
  • Develop and update Plans of Care in accordance with Medicare Conditions of Participation (CoPs)
  • Utilize clinical judgment and understanding of PDGM to support medical necessity and appropriate home health interventions
  • Provide therapeutic exercises, gait training, balance training, strengthening, mobility training, and pain management techniques
  • Educate patients and caregivers on home exercise programs, safety, fall prevention, and overall wellness
  • Collaborate with physicians, nurses, occupational therapists, speech therapists, and other interdisciplinary team members
  • Monitor patient progress, modify treatment plans as needed, and communicate updates appropriately
  • Complete accurate and timely electronic documentation in compliance with agency and regulatory standards
  • Promote patient independence, safety, and improved quality of life within the home setting
