As a Home Health Physical Therapist, you are a critical part of the care team - helping patients safely recover, regain mobility, and maintain independence where they feel most comfortable: at home. Requirements

  • Graduate of an accredited Physical Therapy program (DPT or Master’s)
  • Current PT license in the state of practice
  • Minimum 1–2 years of experience
  • Strong clinical assessment and critical thinking skills
  • Excellent communication and patient education abilities
  • Current CPR certification
  • Valid driver’s license, reliable transportation, and auto insurance

Responsibilities

  • Deliver Patient-Centered Care: Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential. Develop and implement individualized care plans in collaboration with physicians and the care team. Provide hands-on therapy to improve mobility, strength, balance, and independence. Educate patients and caregivers on treatment plans, exercises, and safety strategies.
  • Drive Outcomes & Recovery: Monitor patient progress and adjust treatment plans as needed. Support safe transitions of care and contribute to discharge planning. Identify additional patient needs and collaborate with interdisciplinary teams.
  • Collaborate & Communicate: Partner closely with nurses, therapists, and physicians to ensure coordinated care. Document patient care and progress accurately and timely within the EMR. Participate in case conferences and team meetings.
  • Lead & Support: Supervise Physical Therapist Assistants (PTAs) in accordance with state guidelines. Provide guidance and education to caregivers and support staff.
