Requirements

Graduate of an accredited Physical Therapy program (DPT or Master’s)
Current PT license in the state of practice
Minimum 1–2 years of experience
Strong clinical assessment and critical thinking skills
Excellent communication and patient education abilities
Current CPR certification
Valid driver’s license, reliable transportation, and auto insurance

Responsibilities

Deliver Patient-Centered Care: Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential. Develop and implement individualized care plans in collaboration with physicians and the care team. Provide hands-on therapy to improve mobility, strength, balance, and independence. Educate patients and caregivers on treatment plans, exercises, and safety strategies.
Drive Outcomes & Recovery: Monitor patient progress and adjust treatment plans as needed. Support safe transitions of care and contribute to discharge planning. Identify additional patient needs and collaborate with interdisciplinary teams.
Collaborate & Communicate: Partner closely with nurses, therapists, and physicians to ensure coordinated care. Document patient care and progress accurately and timely within the EMR. Participate in case conferences and team meetings.
Lead & Support: Supervise Physical Therapist Assistants (PTAs) in accordance with state guidelines. Provide guidance and education to caregivers and support staff.
