About The Position
HealthPro Heritage is hiring a Physical Therapist for a per diem opportunity in a senior living community in Buffalo Grove, Illinois. The community provides Assisted Living, Memory Care, and Short-Term Stay services. The therapist will evaluate patients’ physical conditions and provide individualized treatment and education to support recovery, improve functionality, and alleviate pain. The role includes documenting evaluations and progress, coordinating care with other healthcare providers, and participating in multidisciplinary meetings as needed. The stated wage range is $49–$55 per hour.
Requirements
- Degree in Physical Therapy from an accredited institution.
- Valid state licensure as a Physical Therapist, or eligibility for licensure.
- Strong clinical assessment and diagnostic skills.
- Proficiency in developing and implementing effective treatment plans.
- Excellent communication and interpersonal skills with a patient-centered approach.
- Ability to lift and assist patients as needed, stand for extended periods, and perform physical tasks related to therapy.
Responsibilities
- Evaluate patients’ physical conditions, including mobility, strength, and flexibility.
- Provide therapeutic exercises, manual therapy, and other treatments to enhance functionality and alleviate pain.
- Educate patients and families about exercises, posture, body mechanics, recovery, injury prevention, home routines, and lifestyle changes.
- Maintain accurate records of evaluations, progress, and treatment plans in compliance with legal and ethical standards.
- Work with healthcare providers to support effective patient care and participate in multidisciplinary team meetings as needed.
