About The Position

FOX Rehabilitation is seeking licensed Physical Therapists to provide one-on-one care to older adults in private residences and Senior Living Communities in Arlington, Virginia, and surrounding communities. Using FOX’s Geriatric House Calls™ model, clinicians help patients improve mobility, independence, wellness, and quality of life. Care may also include telehealth and other virtual services when appropriate.

The role offers autonomy to create and manage a treatment schedule, build lasting patient relationships, and work across in-home and senior living settings. FOX is a clinician-operated practice focused on evidence-based care, meaningful relationships, and abolishing ageism. Full-time, part-time, and PRN / FOX Flex employment pathways are available; benefits depend on eligibility and employment type. New graduates may be eligible for FOX’s Emerging Professionals Mentor Program, and H1B sponsorship is available for qualified clinicians.

Requirements

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain one.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.
• Ability to meet the physical demands of in-home and community-based care, including lifting, CPR, patient transfers and bed mobility, repositioning patients, extended standing and walking, climbing stairs, and therapeutic activities.
• Ability to drive to patient homes and senior living communities.
• Wear PPE when required.

Responsibilities

• Provide one-on-one physical therapy to older adults in private residences and senior living settings, including independent living, assisted living, and memory care.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence.
• Document care in FOX’s EMR system in accordance with regulatory standards.
• Collaborate with an interdisciplinary team of physical therapists, occupational therapists, speech-language pathologists, and fitness specialists.
• Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
• Provide telehealth or virtual care when appropriate.
• Potentially take on additional opportunities such as supervising assistants, mentoring, teaching, or creating educational and social content.
