About The Position

PruittHealth @ Home is hiring Physical Therapists to provide evaluative, consultative, and treatment services to home health patients in their residences following referral by a licensed physician. The role includes evaluating functional ability and therapy needs, providing treatment to relieve pain and restore or develop function, and supporting the best possible patient outcomes according to the established plan of care. Therapists communicate with patients, families, physicians, home health partners, and interdisciplinary team members. The position involves field visits to evaluate and initiate starts of care and to monitor clinical outcomes and follow-up care. Therapists assess, document, and report patient progress using required records and clinician-preferred technology, complete documentation at the point of care and within established timelines, coordinate care and referrals, and educate patients and families using approved teaching materials. The role also includes meeting productivity standards, participating in agency meetings and quality programs, following safety procedures, maintaining current credentials and clinical records, and adapting to changing workloads. The employer describes a pay-per-point compensation model, with higher point values for visits requiring greater effort. The position offers a progressive benefit plan, mileage reimbursement, career-growth opportunities, additional pay incentives, flexible schedules, and a team environment. A $15,000 sign-on bonus is listed for the Kershaw County opportunity. Full-time openings are listed for Kershaw County and Richland/Lexington Counties.
