Physical Therapist / PT (Home Health)
About The Position
Mission Healthcare is hiring a Physical Therapist to provide home health care in the Greater Murrieta Area. The role involves evaluating patients, developing individualized treatment plans, delivering therapeutic interventions, educating patients and caregivers, coordinating with an interdisciplinary team, and documenting care. The schedule is Monday through Friday, 8 a.m. to 5 p.m. The pay range is $55–$60 per hour, depending on experience. Requirements
- Active Physical Therapist license in the practicing state.
- Master's or Doctorate degree in Physical Therapy.
- At least 1 year of experience as a Physical Therapist.
- Current BLS and hands-on CPR certification.
- Valid driver's license and reliable, insured vehicle.

- Home health experience is strongly preferred.
Responsibilities
- Evaluate and assess patients' physical conditions and develop individualized treatment plans.
- Implement therapeutic exercises and techniques to relieve pain and improve mobility, strength, and coordination.
- Educate patients and caregivers on exercises, safety precautions, and self-care strategies.
- Collaborate with an interdisciplinary team to provide holistic, patient-centered care.
- Maintain accurate and timely documentation of patient progress and treatment plans.
