About The Position
Kace Premier Medical Talent is recruiting a Physical Therapist for a skilled nursing rehabilitation opportunity in West Reading, Pennsylvania. Support patients working toward improved mobility, safer movement, and greater independence.
Requirements
- Graduate of an accredited physical therapy program.
- Current Pennsylvania PT license, or eligibility to obtain licensure before starting.
- Strong assessment, treatment planning, and documentation skills.
- Ability to work effectively with an interdisciplinary team.
Responsibilities
- Perform physical therapy evaluations and identify functional limitations.
- Establish measurable goals and individualized treatment plans.
- Provide therapeutic exercise, gait training, balance training, and functional mobility interventions.
- Monitor outcomes and adjust treatment as appropriate.
- Educate patients and caregivers on safe movement and prescribed exercises.
- Coordinate with rehabilitation and nursing colleagues.
- Complete accurate clinical documentation and support discharge planning.
