About The Position

Stern at Home Therapy is hiring Full-Time, Part-Time, and PRN Physical Therapists to provide one-on-one care to patients in their homes throughout Kapolei and the communities east of Pearl City, HI. Clinicians enjoy the flexibility to create their own schedules while receiving the support of a dedicated administrative team that manages referrals, scheduling, authorizations, and logistics. The focus is on exceptional patient care without productivity quotas, double-booking, or clinic politics.

Requirements

• Active Physical Therapist license in Hawaii required.
• Graduation from an accredited Physical Therapy program.
• Reliable transportation required.
• Strong communication, documentation, and time-management skills.
• Ability to work independently in a home health setting.

Nice To Haves

• Home health or geriatric experience is preferred but not required.

Responsibilities

• Perform physical therapy evaluations and develop individualized plans of care.
• Provide one-on-one physical therapy treatments in patients' homes.
• Address gait training, balance, strengthening, transfers, endurance, pain management, and fall prevention.
• Recommend assistive devices, durable medical equipment, and home safety modifications.
• Complete accurate and timely clinical documentation.
• Collaborate with physicians, caregivers, family members, and the interdisciplinary care team.
• Maintain compliance with all clinical and regulatory requirements.
