Adult Home Care Physical Therapist (PT)
About The Position
BAYADA Home Health Care is seeking a compassionate Physical Therapist (PT) to provide one-on-one, in-home care throughout Union County. This per diem opportunity offers flexible visits, allowing you to control your hours (0-4 per week) without minimums or pressure. You will provide meaningful, one-on-one patient care with manageable caseloads close to home, supported by leadership and clinical resources. The role involves seeing patients in their homes, performing evaluations, developing personalized treatment plans, delivering skilled therapy focused on patient goals and independence, and educating patients and families. Efficient electronic documentation is required.
Requirements
- Licensed Physical Therapist in good standing in New Jersey
- Graduation from an accredited PT program
- Passion for patient-centered, in-home care
- Reliable transportation and willingness to travel locally
Responsibilities
- See patients in their homes throughout Union County
- Perform evaluations and develop personalized treatment plans
