About The Position
We are currently seeking a Physical Therapist (PT) to join our outstanding healthcare team! If you’re a compassionate Physical Therapist (PT) who is passionate about helping patients regain mobility and independence, we’d love to meet you. In this role, you will build meaningful relationships with patients and families while promoting independence, improving outcomes, and ensuring compassionate, patient-centered care.
Requirements
- Current California Physical Therapist license
- Current BLS certification
- Valid driver’s license, reliable transportation, and auto insurance
- Strong clinical assessment and treatment planning skills
- Ability to work independently while collaborating with an interdisciplinary team
- OASIS documentation experience
Nice To Haves
- Previous experience working in home health, hospice, or a similar caregiving environment
- Familiarity with KanTime Electronic Medical Record (EMR) system
- Bilingual skills are a plus
Responsibilities
- Evaluate patients
- Develop individualized treatment plans
- Improve strength, balance, mobility, and functional independence
- Educate patients and caregivers on safety and fall prevention
- Collaborate with physicians and the interdisciplinary team to maximize patient outcomes
- Help patients safely recover and remain independent in their own homes
