As a Registered Nurse (RN), you will provide skilled nursing care to patients in the comfort of their homes, develop individualized care plans, perform comprehensive assessments, administer medications and treatments, educate patients and caregivers, and coordinate care with physicians and the interdisciplinary team. You will play a critical role in helping patients recover safely, manage chronic conditions, and avoid unnecessary hospitalizations. In this role, you will build meaningful relationships with patients and families while promoting independence, improving outcomes, and ensuring compassionate, patient centered care.

Requirements

  • Current California Registered Nurse (RN) license
  • Current BLS certification
  • Valid driver’s license, reliable transportation, and auto insurance
  • Excellent assessment, communication, and critical thinking skills
  • Ability to work independently while collaborating with an interdisciplinary team
  • OASIS documentation experience

Nice To Haves

  • Previous experience working in home health, hospice, or a similar caregiving environment
  • Familiarity with KanTime Electronic Medical Record (EMR) system
  • Bilingual skills are a plus
