About The Position

HealthPro Heritage is hiring a Home Health Physical Therapist to provide care to adult patients in Wallingford, Connecticut, and surrounding areas. The role supports the organization’s partnership with Masonicare Home Health and includes visits both on and off the Ashlar Village campus. A full-time schedule is available from Saturday through Tuesday. The Physical Therapist evaluates patients’ physical conditions and provides treatment to improve function and relieve pain. The role also includes educating patients and families, maintaining accurate clinical documentation, and coordinating care with other healthcare providers. Requirements

Degree in Physical Therapy from an accredited institution.
Valid state Physical Therapist license or eligibility for licensure.
Strong clinical assessment and diagnostic skills.
Ability to develop and implement effective treatment plans.
Excellent communication and interpersonal skills, with a patient-centered approach.
Ability to lift and assist patients as needed, stand for extended periods, and perform physical tasks related to therapy.

Responsibilities

Evaluate patients’ physical conditions, including mobility, strength, and flexibility.
Provide therapeutic exercises, manual therapy, and other treatments to improve functionality and alleviate pain.
Educate patients and families about exercises, posture, body mechanics, home routines, and lifestyle changes to support recovery and prevent injuries.
Maintain accurate records of evaluations, progress, and treatment plans in accordance with legal and ethical standards.
Collaborate with healthcare providers to support effective patient care and participate in multidisciplinary team meetings as needed.
