About The Position

This role offers a unique opportunity for a Physical Therapist (PT) to transition from the traditional clinic setting to home health. Unlike the fast-paced clinic environment where therapists often see patients every 30 minutes, this position allows for focused, one-on-one treatment in patients' homes. The focus is on evaluating a patient's specific limitations to independence, developing personalized treatment plans, and educating patients and caregivers. This approach emphasizes achieving functional goals in the patient's natural environment, such as safely getting out of bed, navigating stairs, or walking to the kitchen. The role provides a high degree of autonomy and independence, allowing therapists to utilize their clinical judgment to help patients regain and maintain their independence at home.

Requirements

  • Active Minnesota Physical Therapist (PT) license
  • Strong clinical reasoning and assessment skills
  • Excellent patient and caregiver communication
  • Confidence working independently
  • Ability to organize and manage a field-based schedule
  • Passion for improving mobility and functional independence
  • Valid driver's license and reliable transportation
  • Willingness to travel throughout your assigned Twin Cities territory

Nice To Haves

  • Home health experience is valuable, but not required. Experience in outpatient therapy, inpatient rehabilitation, skilled nursing, TCU, acute care, or another clinical environment is also considered.

Responsibilities

  • Complete comprehensive Physical Therapy evaluations in patients' homes
  • Develop individualized plans of care and functional goals
  • Improve mobility, strength, balance, endurance, transfers, and gait
  • Help reduce fall risk and improve home safety
  • Provide therapeutic exercise and functional mobility training
  • Educate patients and caregivers on home exercise programs and safe mobility techniques
  • Recommend strategies and equipment to improve independence
  • Monitor patient progress and adjust treatment plans as needed
  • Collaborate with physicians, nurses, occupational therapists, speech therapists, and other interdisciplinary clinicians
  • Complete timely and accurate clinical documentation
  • Help patients safely regain independence and remain in their homes
