About The Position
This position offers a unique opportunity for a Physical Therapist (PT) to transition from a traditional clinic setting to home health care. The role emphasizes one-on-one patient care, allowing therapists to focus on individual needs and functional goals within the patient's home environment. Unlike the fast-paced clinic grind, this role provides greater independence, autonomy, and the ability to see the direct impact of therapy on a patient's daily life. Requirements
- Active Minnesota Physical Therapist (PT) license
- Strong clinical reasoning and assessment skills
- Excellent patient and caregiver communication
- Confidence working independently
- Ability to organize and manage a field-based schedule
- Passion for improving mobility and functional independence
- Valid driver's license and reliable transportation
- Willingness to travel throughout your assigned Twin Cities territory
Nice To Haves
- Home health experience is valuable
- Experience developed in outpatient therapy, inpatient rehabilitation, skilled nursing, TCU, acute care, or another clinical environment
Responsibilities
- Complete comprehensive Physical Therapy evaluations in patients' homes
- Develop individualized plans of care and functional goals
- Improve mobility, strength, balance, endurance, transfers, and gait
- Help reduce fall risk and improve home safety
- Provide therapeutic exercise and functional mobility training
- Educate patients and caregivers on home exercise programs and safe mobility techniques
- Recommend strategies and equipment to improve independence
- Monitor patient progress and adjust treatment plans as needed
- Collaborate with physicians, nurses, occupational therapists, speech therapists, and other interdisciplinary clinicians
- Complete timely and accurate clinical documentation
- Help patients safely regain independence and remain in their homes
