About The Position

FOX Rehabilitation is seeking licensed Physical Therapists to provide one-on-one care to older adults in private residences and senior living communities in Wheaton, Maryland, and surrounding communities. Using the Geriatric House Calls™ model, clinicians help patients improve mobility, independence, wellness, and quality of life. Care settings include independent living, assisted living, and memory care communities. Clinicians may also incorporate telehealth and other virtual care into their schedules. The role offers autonomy to create, manage, and adjust a treatment schedule and develop lasting patient relationships. FOX is a clinician-operated practice focused on evidence-based care, meaningful relationships, and a mission to abolish ageism. Available employment pathways include full-time, part-time, and PRN/FOX Flex positions. New graduates may be eligible for the Emerging Professionals Mentor Program, and H1B sponsorship is available for qualified clinicians. The standard Maryland base pay range for a full-time role is $70,000–$110,000 annually, with opportunities to earn more. Part-time and FOX Flex compensation is prorated. Eligible-role benefits include medical, dental, and vision insurance, a 401(k) with company match, unlimited continuing education access, professional certification reimbursement, mileage reimbursement, and student loan repayment for those who qualify.

Requirements

Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain one.
Degree from an accredited Physical Therapy program.
Basic computer and EMR proficiency.
Current CPR certification.
Ability to meet the physical demands of in-home and community-based care, including lifting, performing CPR, assisting with patient transfers and bed mobility, repositioning patients, standing and walking for extended periods, climbing stairs, and performing therapeutic activities.
Ability to drive to patient homes and senior living communities.
Wear PPE when required.

Responsibilities

Provide one-on-one physical therapy to older adults in private residences and senior living communities, including independent living, assisted living, and memory care.
Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
Provide therapy sessions that support patient wellness, longevity, mobility, and functional independence.
Document effectively in FOX’s EMR system in accordance with regulatory standards.
Collaborate with an interdisciplinary team of physical therapists, occupational therapists, speech-language pathologists, and Fitness Specialists.
Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
Provide telehealth or virtual care when appropriate.
Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
