Contract Physical Therapist (Adult & Pediatric Home Health)
About The Position
We are a home health agency deeply committed to quality patient care, flexibility, and therapist autonomy. We are seeking a Full-Time Physical Therapist who is passionate about serving both adult and pediatric patients and thrives in a growth-oriented environment. This position is ideal for therapists who value independence, flexible scheduling, and putting patients first. The Physical Therapist is responsible for delivering high-quality physical therapy services in accordance with physician orders and agency standards. The therapist will assess patient needs, develop individualized plans of care, and provide skilled interventions to improve mobility, strength, function, and overall quality of life.
Requirements
- Active Texas Physical Therapy license
- Experience with both adult and pediatric patients
- Familiarity with EMR systems (Kantime experience a plus; training provided)
- Strong clinical judgment and patient-centered mindset
- Active Physical Therapy License (TX)
- CPR/BLS Certification
- TB Test or Chest X-ray
- Clear background check

Responsibilities
- Evaluate patients and develop individualized Plans of Care (POC)
- Perform SOC, Recertifications, Evaluations, and routine visits
- Implement treatment plans to improve strength, ROM, flexibility, and pain management
- Complete timely and accurate clinical documentation
- Communicate effectively with physicians, caregivers, and the care team
- Adhere to all BrightStar Care standards, safety protocols, and regulations
- Travel within an established service radius
- Maintain compliance with all local, state, and federal regulations
