About The Position

Iowa Home Care is seeking a passionate and skilled Physical Therapist to join their team in the Des Moines area. This role focuses on providing in-home evaluations, treatments, and education to help patients improve strength, mobility, safety, and independence. The Physical Therapist will collaborate with an interdisciplinary team, including nurses, occupational therapists, and speech therapists, to deliver comprehensive, patient-centered care. Both full-time and part-time positions are available, offering the opportunity to make a significant difference in the lives of Iowans by enabling them to remain safely in their homes.

Requirements

  • Licensed to practice as a Physical Therapist in the state of Iowa
  • Minimum of two years of experience in physical therapy
  • Strong communication, organizational, and problem-solving skills
  • Valid driver’s license with reliable transportation

Nice To Haves

  • Community/home health experience preferred

Responsibilities

  • Conduct comprehensive in-home physical therapy evaluations
  • Develop and implement individualized treatment plans focused on mobility, strength, balance, and safety
  • Provide therapeutic interventions including exercise programs, gait training, transfer training, and pain management
  • Educate patients and caregivers on home exercise programs, safety strategies, and equipment use
  • Collaborate with nurses, occupational therapists, speech therapists, social workers, and case managers to coordinate care
  • Document all patient visits, progress notes, and care plans in compliance with agency and regulatory standards
  • Recommend adaptive equipment and home modifications when appropriate
  • Monitor patient progress and adjust treatment plans as needed
  • Communicate changes in condition or concerns to the interdisciplinary care team
  • Promote independence and ongoing improvement through compassionate, patient-centered care
