This role involves assisting in planning, organizing, developing, and implementing Physical Therapy services in accordance with current applicable federal, state, and local standards, guidelines and regulations. The Physical Therapist must possess the ability to plan, develop, organize, evaluate, and implement the programs, goals, objectives, policies and procedures, etc., of the physical therapy program to help residents obtain/maintain their highest level of physical, mental and social well-being. The Physical Therapist will interpret and/or instruct in physical therapy policies and procedure to personnel, residents, family members, etc. as necessary. The Physical Therapist will participate in discharge planning, development and implementation of resident care plans, resident assessments (MDS), etc. as necessary. This role also includes supervising Physical Therapy Assistants as outlined by the State Physical Therapy Practice Act and maintaining treatment records, resident files, and progress notes as required and maintaining an adequate liaison with families and residents.
