About The Position

FOX Rehabilitation is seeking a licensed Physical Therapist to provide one-on-one care to older adults in Avon, Indiana and surrounding communities. Care is delivered in private residences and senior living settings, including independent living, assisted living, and memory care, through FOX’s Geriatric House Calls™ model. Clinicians help patients improve mobility, independence, wellness, and quality of life using evidence-based treatment. Telehealth and other virtual care may also be incorporated when appropriate.

The role offers autonomy to create and manage a treatment schedule and build lasting patient relationships within a clinician-operated, interdisciplinary practice. FOX offers full-time, part-time, and PRN/Fox Flex pathways; this posting identifies the opportunity as part-time. Benefits are available to eligible roles, with part-time benefits eligibility based on minimum hours requirements. New graduates may be eligible for the Emerging Professionals Mentor Program, and H1B sponsorship is available for qualified clinicians.

Requirements

• Hold a valid Physical Therapy license in the state(s) of practice, or be eligible to obtain one.
• Have a degree from an accredited Physical Therapy program.
• Have basic computer and EMR proficiency.
• Hold current CPR certification.
• Be able to meet the physical demands of in-home and community-based care, including lifting, performing CPR, assisting with patient transfers and bed mobility, repositioning patients, standing and walking for extended periods, climbing stairs, and performing therapeutic activities.
• Wear PPE when required.

Responsibilities

• Provide one-on-one physical therapy to older adults in private residences and senior living communities, including independent living, assisted living, and memory care settings.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Provide therapy that supports patient wellness, longevity, mobility, and functional independence.
• Document care in FOX’s EMR system in accordance with regulatory standards.
• Collaborate with an interdisciplinary team of physical therapists, occupational therapists, speech-language pathologists, and Fitness Specialists.
• Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
• Provide telehealth or virtual care when appropriate.
• Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
