Requirements

  • A graduate of a PT Program recognized by the American Physical Therapy Association or other National accepted Council or Association
  • An active license to practice Physical Therapy in MN
  • A minimum of four (4) years’ experience as a Physical Therapist, home care experience a plus
  • Must have criminal background check
  • Must have current CPR certification
  • Must have active driver's license and clear driving record
  • Computer skills.
  • Ability to exercise initiative and independent judgment

Responsibilities

  • Assessment, planning, implementation, and evaluation of all physical therapy modalities to patients.
  • All duties and standards will be performed according to established policies, procedures, and competency guidelines within the department and organization.
  • Improves or minimizes residual physical disabilities of the patient.
  • Returns the individual to optimum and productive level within patient’s capabilities.
  • Participates with all other home care personnel in patient care planning.
  • Takes initial history and makes initial evaluation.
  • Consults with physicians regarding patient’s progress
  • Participates in quality assessment and performance improvement activities as assigned.
  • Prepares clinical and progress notes.
  • Helps develop the plan of care and revise, as necessary.
  • Consults with family and agency personnel.
