About The Position

Mass General Brigham Home Care provides post-acute, medically necessary skilled care in the home to help homebound patients recover from illness, surgery, or injury and manage exacerbations of chronic disease. Our clinicians are highly skilled professionals who function independently while collaborating as part of a strong interdisciplinary team to deliver the highest quality patient-centered care. As a Physical Therapist on our Weekend team, you will perform Start of Care's, Resumption of Care's or Evaluations. You will evaluate patients and develop personalized treatment plans, focusing on mobility, strength, fall prevention, and functional independence. You will assess home environments to promote safety and independence, educate patients and caregivers on recovery strategies and long-term mobility, and collaborate with nurses, therapists, physicians, and social workers to support recovery. You won’t just treat symptoms — you’ll help patients regain independence and confidence in their own environment. Most of your time will be spent South of Boston supporting the following communities: Mansfield • North Attleboro • Taunton • Walpole • Brockton • Marshfield • Plymouth • Weymouth and surrounding town. While navigating transportation and parking in the city can be challenging, we will secure city parking passes for new team members to make the commute as convenient and stress-free as possible!

Requirements

  • One year of experience in acute care, rehab or homecare as a Physical Therapist
  • Registered with the Board of Registration in Massachusetts.
  • Excellent verbal, written and electronic communications skills
  • Computer proficiency and ability to learn new software
  • Ability to prioritize work and adapt to changing situations
  • Travel required within Mass General Brigham Healthcare at Home’s geographic area

Responsibilities

  • Perform Start of Care's, Resumption of Care's or Evaluations
  • Evaluate patients and develop personalized treatment plans
  • Focus on mobility, strength, fall prevention, and functional independence
  • Assess home environments to promote safety and independence
  • Educate patients and caregivers on recovery strategies and long-term mobility
  • Collaborate with nurses, therapists, physicians, and social workers to support recovery
  • Provide physical therapy services to patients in their place of residence who have physical impairments, functional limitations or disabilities resulting from injury, disease or other causes.
  • Develop a plan of care that is patient and family focused and institutes standardized interventions for goal-oriented outcomes
  • Daily documentation is accurate and timely
  • Application of standard treatment procedures
  • Teaching is provided to patients and caregivers
