About The Position

Human Touch Home Health is seeking a compassionate and motivated Physical Therapist (PT) to join our growing team serving patients throughout Washington, DC. This full-time opportunity offers professional independence, meaningful one-on-one patient care, and the support of an experienced clinical team. As a Home Health Physical Therapist, you'll provide individualized care in the comfort of your patients' homes, helping them regain strength, restore mobility, improve balance, and return to the activities that matter most. You'll have the autonomy to develop personalized treatment plans while building meaningful relationships with patients and their families. This role focuses on purposeful therapy, meaningful connections, and the opportunity to change lives one visit at a time, without crowded therapy gyms or double-booked appointments.

Requirements

Active Washington, DC Physical Therapist License Required
Valid Driver's License and Reliable Transportation
Strong Clinical Assessment and Documentation Skills
Excellent Communication and Interpersonal Skills
Passion for Delivering Exceptional Patient-Centered Care

Nice To Haves

Home Health Experience Preferred but Not Required
OASIS Experience is a Plus

Responsibilities

Complete evaluations, reassessments, and discharge visits
Develop individualized plans of care based on patient goals and clinical needs
Provide skilled physical therapy services in patients' homes
Educate patients and caregivers on therapeutic exercises, mobility training, fall prevention, and home safety
Monitor patient progress and modify treatment plans as needed
Collaborate with physicians and the interdisciplinary care team
Complete timely and accurate clinical documentation using WellSky
