Requirements
- Active Virginia Physical Therapist License Required
- Minimum of One Year of Physical Therapy Experience
- OASIS Experience Preferred but Willingness to Learn Required
- Valid Driver's License and Reliable Transportation
- Comfortable Working Independently in Patients' Homes
Nice To Haves
- Home Health Experience Preferred but Not Required
Responsibilities
- Complete physical therapy evaluations, reassessments, and discharge visits
- Develop individualized plans of care based on patient goals
- Provide skilled physical therapy services in patients' homes
- Complete Starts of Care and ongoing patient visits
- Educate patients and caregivers on therapeutic exercises, mobility training, fall prevention, and home safety
- Collaborate with physicians and the interdisciplinary care team
- Complete documentation electronically using WellSky on a company-issued tablet
