About The Position

H2 Health Prime Living is seeking a Full-Time Physical Therapist to provide 1:1 patient care within luxury Senior Living communities. This role offers a flexible schedule and focuses on treating patients with orthopedic, post-operative, neurological, and sports-related conditions in an outpatient setting. The ideal candidate will create personalized treatment plans, document patient progress using EMR, and collaborate with a multidisciplinary team to deliver holistic, patient-centered care. Requirements

• Must have a degree from an accredited Physical Therapy program.
• An active or eligible state license to practice as a Physical Therapist (PT)
• New graduates are welcome!
• Outpatient experience is a plus, but we value passion and a commitment to patient care.
