AdvantageCare Rehabilitation is looking for a PRN Physical Therapist (PT) to join our rehabilitation team at Grand Rapids Care Center in Grand Rapids, Ohio. Requirements
- Graduate of an accredited Physical Therapy program
- Current or eligible Ohio Physical Therapist license
- Strong clinical judgment and communication skills
- Ability to work collaboratively with PTAs and the interdisciplinary team
- A patient-first approach to care
Nice To Haves
- Skilled nursing, geriatric or rehabilitation experience is a plus
Responsibilities
- Performing comprehensive Physical Therapy evaluations
- Developing individualized plans of care based on patient needs and goals
- Addressing strength, balance, gait, transfers, endurance and functional mobility
- Providing therapeutic exercise, neuromuscular re-education and functional mobility training
- Assessing the need for assistive devices and providing appropriate training
- Developing fall-prevention and safety strategies
- Monitoring patient progress and modifying treatment plans when appropriate
- Educating patients, families and caregivers
- Collaborating with PTAs and the interdisciplinary care team
- Completing accurate and timely clinical documentation
- Helping patients work toward greater safety, mobility and independence
