Requirements

  • Graduate from an accredited physical therapy program Required
  • Bachelor's Degree in Physical Therapy Required or Master's Degree in Physical Therapy Required or Doctorate in Physical Therapy Required
  • Physical Therapist 1.00 Required
  • Basic Life Support 2.00 Required
  • Effective communication skills, customer service focused, team oriented, and basic computer skills.

Responsibilities

  • Collaborate with physicians to develop, evaluate, and update treatment programs, care plans, goals, and discharge plans based on the patient’s physical therapy needs.
  • Create and adjust care plans with input from the patient and/or family.
  • Develop realistic discharge plans incorporating patient and family goals.
  • Implement and adjust physical therapy treatment plans.
  • Select appropriate activities to help patients achieve their goals.
  • Revise goals and plans with input from patients and families.
  • Provide ongoing education and training to patients and families.
  • Give discharge instructions and referrals to community resources as needed.
  • Serve as a resource for staff, families, and physicians regarding rehabilitation services and equipment.
  • Document evaluations, treatment plans, and each session according to departmental guidelines.
  • Complete and submit all required documentation, including family education and discharge information.
  • Uphold ethical standards and demonstrate integrity and honesty.
  • Embrace diversity and collaborate effectively.
  • Maintain confidentiality of patient information.
  • Adhere to attendance, punctuality, and dress code policies, including wearing an ID badge.
  • Communicate clearly and effectively with all team members.
