Requirements

Graduate of an accredited Physical Therapy program
Active Indiana Physical Therapy license

Responsibilities

Perform patient evaluations and physical therapy assessments
Review medical history, imaging, and physician referrals
Develop individualized treatment plans based on patient needs and goals
Guide patients through therapeutic exercises, modalities, mobility training, and rehabilitation programs
Educate patients on treatment plans, home exercises, posture, stretching, and lifestyle modifications
Monitor patient progress and perform reassessments
Maintain accurate and timely EMR documentation
Collaborate with physicians and multidisciplinary providers to ensure optimal patient outcomes
