About The Position

HTS is seeking a full-time Physical Therapist to join the therapy teams at two of our partner continuing care retirement communities in Fort Wayne, Indiana, and Allen County. Scheduling can be very flexible and creative and is based on caseload and staffing needs at the assigned facilities. This is a unique opportunity to experience a variety of work environments, team dynamics and patient populations. The role involves floating between two sites daily, which are 10 miles/15 minutes apart, with compensated drive time and mileage for qualified travel. Facilities offer diverse patient populations from short-term rehab, long term care, outpatient, memory care, assisted living and independent living. The organization is a therapist-owned company that values clinical excellence and provides a supportive environment.

Requirements

• Active Indiana Physical Therapy license (or eligible to obtain).
• Graduate of a CAPTE-accredited Physical Therapy program.
• Passion for working with older adults and improving quality of life.
• Strong communication and collaboration skills.
• Must hold an active Indiana Physical Therapist license or have the necessary qualifications to earn Indiana license.
• Qualifications include earning an advanced degree from a Commission on Accreditation in Physical Therapy Education-accredited physical therapist education program, passing the state licensure exam, and fulfilling the state’s requirements for licensure.

Responsibilities

• Provide personalized therapy to residents to help them thrive.
• Float between two continuing care retirement communities daily.
• Work with diverse patient populations including short-term rehab, long-term care, outpatient, memory care, assisted living, and independent living.
