About The Position

We are seeking a compassionate and motivated Physical Therapist for a Skilled Nursing Facility setting. This opportunity offers the chance to work closely with patients to improve mobility, strength, and overall quality of life.

Requirements

• Active Physical Therapist (PT) license in Arizona or compact eligibility required
• Graduate of an accredited Physical Therapy program required
• Strong communication, patient care, and clinical assessment skills required
• Ability to work independently and collaboratively within a multidisciplinary healthcare team required
• Excellent organizational and documentation skills required

Nice To Haves

• Current CPR/BLS certification preferred
• Minimum 1 year recent Physical Therapist experience preferred
• Skilled Nursing Facility (SNF) or rehabilitation experience preferred

Responsibilities

• Evaluate patients and develop individualized physical therapy treatment plans
• Provide therapeutic exercises, mobility training, and rehabilitation services to patients recovering from injury, illness, or surgery
• Monitor patient progress and adjust treatment plans as needed to achieve rehabilitation goals
• Educate patients and family members on therapy programs, exercises, and injury prevention techniques
• Collaborate with physicians, nurses, and interdisciplinary healthcare teams to coordinate patient care
• Maintain accurate and timely documentation of patient treatments, progress notes, and discharge planning
• Assist patients in improving strength, balance, flexibility, mobility, and functional independence
• Ensure compliance with facility policies, patient safety standards, and healthcare regulations
• Participate in care planning meetings and contribute to quality patient outcomes
