About The Position

Full-Time Physical Therapist Site Lead We are seeking a motivated and compassionate Physical Therapist Site Lead to join our growing rehabilitation team. This full-time leadership role is ideal for a PT who enjoys combining patient care with mentorship, collaboration, and program development in a supportive skilled nursing/rehab environment. As a Site Lead, you will play a key role in driving clinical excellence, supporting team development, and enhancing resident outcomes.

Requirements

• Licensed Physical Therapist in good standing
• Strong communication and organizational skills
• Passion for patient-centered care and team collaboration
• Ability to manage responsibilities across multiple sites

Nice To Haves

• Previous leadership or supervisory experience preferred, not required

Responsibilities

• Deliver high-quality physical therapy services to residents
• Serve as the primary rehabilitation communicator with the interdisciplinary team (IDT)
• Participate actively in daily clinical and operational meetings
• Support and mentor therapy staff, including PTA supervision
• Promote a positive, respectful, and collaborative team culture
• Identify opportunities to improve therapy services and resident outcomes
• Assist with census growth initiatives and strengthen community relationships
• Collaborate with leadership to support operational and clinical goals across both locations
