About The Position
Home Health Physical Therapist (PT) PRN. Make a Difference Where Patients Need You Most — At Home. As a Home Health Physical Therapist, you'll develop individualized treatment plans, provide hands-on care, and build meaningful relationships with patients and families while enjoying the flexibility and autonomy that comes with community-based care.
Requirements
- Graduate of an accredited Physical Therapy program.
- Current Physical Therapist license.
- CPR certification.
- Valid driver's license and auto insurance.
Nice To Haves
- Home Health experience preferred.
- HomeCare HomeBase experience preferred.
Responsibilities
- Evaluate patients and develop personalized treatment plans.
- Improve mobility, strength, balance, and overall function.
- Educate patients and caregivers on exercises, safety, and disease management.
- Monitor progress and adjust treatment plans to maximize outcomes.
- Collaborate with physicians, nurses, and interdisciplinary team members.
- Complete timely and accurate clinical documentation.
