About The Position

Empath Home Health, a member of Empath Health, is seeking a compassionate PRN Home Health PT Physical Therapist to provide exceptional and individualized care in Manatee county (Bradenton, Lakewood Ranch, and surrounding areas). Empath Home Health delivers high-quality, outcomes-driven care trusted by patients, families, and healthcare professionals. Empath Home Health provides skilled nursing and therapy services to help patients recover and maintain independence at home, along with personal care support that assists with daily needs as part of a seamless continuum of care. Home health visits are in Manatee county (including Bradenton, Lakewood Ranch, Anna Maria Island, Holmes Beach, Ellenton, Palmetto, and surrounding areas). Visits may also be available in Sarasota and/or Hillsborough counties if desired.

Requirements

• Active Florida PT license
• Minimum of 1 year experience as a licensed Physical Therapist (PT).
• Willingness to participate in weekend and holiday on-call rotations

Nice To Haves

• Home health care experience strongly preferred.
• Prefers experience using Homecare Homebase (HCHB) EMR
• Strong assessment, critical thinking, and patient education skills

Responsibilities

• Perform comprehensive patient assessments, including OASIS assessments at required intervals
• Develop, initiate, and revise individualized plans of care as patient needs evolve
• Deliver skilled physical therapy services and treatments as outlined in the plan of care
• Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies
• Collaborate with the interdisciplinary team to ensure seamless, holistic care
