About The Position
Sage Group HealthCare Staffing is seeking a Physical Therapist (PT) for a Home Health position in Walla Walla, Washington. This is a 13-week travel/contract position, and both local and traveler candidates are welcome. The role involves providing Physical Therapy services to home health patients, consulting with physicians and an interdisciplinary team, and developing treatment plans. The PT will utilize their skills in patient evaluation and treatment to help patients achieve maximum independence. The job also includes driving to patient homes within assigned territories, completing charting and documentation, and assisting patients and their families in maintaining dignity and independence.
Requirements
- Physical Therapist: 1-2 years - Required
- Recent/Current Home Health PT experience: 1 year within the last 3 years - Required
- Home Care Home Base (HCHB) experience - Required
- Must have valid Driver’s License and Personal Auto Insurance
- Degree in Physical Therapy - Required
- PT License for State applying or Compact if applicable - Required
- BLS Certification from AHA or American Red Cross - Required
Nice To Haves
- OASIS experience - Preferred
Responsibilities
- Provide Physical Therapy services for Home Health Patients
- Consult with the physician and an interdisciplinary team in the development of a plan of treatment
- Utilize skills in patient/client evaluation and treatment, professional consultations to achieve maximum patient/client independence in accordance with the individual's functional level and potential
- Plans and prepares treatment plan based on evaluation of patient data
- Provide in-home PT treatment programs to assist patients in restoring function, relieving pain, and preventing disability
- Allow patients and their families to maintain dignity and independence while receiving safe, effective rehabilitation in the security of their home
- Drive to patient homes in the assigned territories on a daily basis.
- Complete charting and patient documentation in timely manner as per client policies and procedures
