About The Position

Empath Home Health, a member of Empath Health, is seeking a compassionate PRN Home Health PT (Physical Therapist) to provide exceptional and individualized care in the Port Charlotte/North Port area. Empath Home Health delivers high-quality, outcomes-driven care trusted by patients, families, and healthcare professionals. They provide skilled nursing and therapy services to help patients recover and maintain independence at home, along with personal care support that assists with daily needs as part of a seamless continuum of care.

Requirements

• Active Florida PT license
• Minimum of 1 year experience as a licensed Physical Therapist (PT).
• Willingness to participate in weekend and holiday on-call rotations
• Strong assessment, critical thinking, and patient education skills

Nice To Haves

• Home health care experience strongly preferred.
• Prefers experience using Homecare Homebase (HCHB) EMR

Responsibilities

• Perform comprehensive patient assessments, including OASIS assessments at required intervals
• Develop, initiate, and revise individualized plans of care as patient needs evolve
• Deliver skilled physical therapy services and treatments as outlined in the plan of care
• Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies
• Collaborate with the interdisciplinary team to ensure seamless, holistic care
