They are growing in Cumming, Georgia and the surrounding area and are looking to add a Physical Therapist PRN to their clinical home-health field team. As a home health Physical Therapist, the role involves conducting one-on-one patient care and patient/family teaching in the home setting. The therapist will work directly with patients in their homes, assessing their needs, developing personalized treatment plans, and implementing therapeutic interventions to optimize their functional abilities.

Requirements

• Graduate of an accredited physical therapy educational program
• Current Physical Therapist / PT state license within the state of practice
• Current CPR certification
• Excellent organizational, time-management, written and verbal communication skills
• Reliable transportation, valid driver’s license and current automobile insurance

Responsibilities

• Conducting one-on-one patient care and patient/family teaching in the home setting
• Working directly with patients in their homes, assessing their needs
• Developing personalized treatment plans
• Implementing therapeutic interventions to optimize patients' functional abilities
