About The Position

At Health Link Home Health & Hospice, Physical Therapists are trusted clinicians who manage their patients, design plans of care, and practice independently, with an experienced team available for support. For over 20 years, Health Link has served the Bay Area with strong systems, consistent referrals, and a clinician-first culture. The company is strengthening its East Bay team and is seeking a Physical Therapist who desires flexibility, independence, modern tools, and an organized home health structure. Requirements

• Current California Physical Therapist license
• Valid California driver’s license
• Strong clinical judgment, organization, and time-management skills

Nice To Haves

• Home health experience preferred
• Strong outpatient, inpatient, or rehab PTs are encouraged to apply

Responsibilities

• Independently perform physical therapy evaluations and reassessments
• Develop and manage individualized plans of care
• Deliver skilled therapy focused on mobility, function, fall prevention, and independence
• Supervise and collaborate with PTAs as appropriate
• Communicate directly with physicians and the interdisciplinary team
• Educate patients and caregivers
• Complete documentation in Kinnser, supported by AI tools
• Serve as the primary PT voice in patient care planning
