About The Position

Responsible for planning and administering physical therapy treatment plans designed to promote overall fitness and health of patients. Candidates must reside within NorthLakes' service area at the time of hire and maintain residency throughout employment. This role involves consulting with and evaluating patients to determine their goals and needs, diagnosing and treating movement dysfunction, and creating plans of care. Documentation of patient records must be in accordance with regulatory, licensing, payor, and accrediting requirements. The therapist will also provide education on therapy exercise techniques and the use of adaptive equipment, administer hands-on therapy, and collaborate with other healthcare providers. Maintaining equipment and ensuring a clean, safe work environment are also key responsibilities.

Requirements

  • Masters Degree in Physical Therapy from an accredited physical therapy school
  • 1-2 years of prior experience in a similar role
  • Successful passing of the National Physical Therapy Examination
  • Active Wisconsin Physical Therapy license
  • Reside within NorthLakes' service area at the time of hire and maintain residency throughout employment.

Nice To Haves

  • Electronic Medical Record experience preferred.

Responsibilities

  • Consult with and evaluate the patient to determine patient goals and needs.
  • Diagnose and treat movement dysfunction.
  • Create plans of care and document patient records in accordance with regulatory, licensing, payor, and accrediting requirements.
  • Provide education on therapy exercise techniques and use of adaptive equipment such as walkers or wheelchairs.
  • Provide hands-on therapy such as massage, thermotherapy, dry needling, etc. as applicable.
  • Work collaboratively with other providers including other therapists, surgeons, and the patient’s primary care provider.
  • Clean and maintain equipment to ensure clean and safe work environment.
