Requirements

Graduate of an accredited Physical Therapy program (for PT)
Active Physical Therapist license in the State of Michigan (for PT)
BLS certification within 3 months of hire (for PT)
Graduate of an accredited Physical Therapist Assistant program (for PTA)
Active Physical Therapist Assistant license in the State of Michigan (for PTA)
BLS certification within 3 months of hire (for PTA)
Strong communication and interpersonal skills
Team-oriented mindset with a patient-first approach
Comfort using electronic health records and basic computer applications

Responsibilities

Provide high-quality outpatient physical therapy services to patients with orthopedic, neurologic, and musculoskeletal conditions
Deliver individualized treatment focused on restoring function, improving strength, relieving pain, and enhancing independence
Educate patients, families, and caregivers throughout the plan of care
Collaborate with an interdisciplinary care team to support patient goals
Perform evaluations and interpret referrals (for PT)
Develop, implement, and adjust treatment plans and goals (for PT)
Establish functional baselines and track patient progress (for PT)
Supervise PTAs, students, and rehab support staff as applicable (for PT)
Participate in program development, process improvement, and clinical education (for PT)
Provide treatment under the direction of a licensed Physical Therapist (for PTA)
Carry out established plans of care and therapeutic interventions (for PTA)
Support patient participation in individual and group therapy activities (for PTA)
Contribute to interdisciplinary collaboration and program initiatives (for PTA)
Document evaluations, treatments, and progress in the electronic health record within compliance timelines
Communicate barriers to timely documentation when needed
Participate in department initiatives, committees, and quality improvement efforts
Serve as a clinical resource within areas of expertise
