About The Position

The Physical Therapist ("Therapist") under minimal supervision, assesses, plans, organizes, and participates in rehabilitative treatments that improve mobility, relieve pain, increase strength, and improve or correct disabling conditions resulting from disease or injury. The Therapist coordinates care of patients in collaboration with healthcare professionals, family and caregiver. Utilizes experience and education to assist with developing programs, improving performance, and studentand workforce education. The Therapist is a subject matter expert that relies on clinical reasoning in synthesizing complex evaluations and routinely collaborates with colleagues and key stakeholders to coordinate patient treatment that is safe, efficient and high-quality.

Requirements

  • Master's Degree required
  • Less than 1 year previous experience in PT; new grads and newly licensed PTs are able to apply for and be hired into Rehab Services Physical Therapist positions required
  • BLS-Basic Life Support (BLS/CPR) - AHA - American Heart Association or American Red Cross required
  • PTL-Physical Therapist License - DORA - Department of Regulatory Agencies required
  • Knowledge of theory, principles and practice of physical therapy.

Responsibilities

  • Deliver, assess, plan, organize, and participate in rehabilitative treatments that improve mobility, relieve pain, increase strength, and improve or correct disabling conditions resulting from disease or injury utilizing evidence-based occupational therapy treatment interventions.
  • Perform and document an initial exam, evaluating data to identify problems and determine a diagnosis prior to intervention.
  • Develop and maintain the patient’s plan of care in collaboration with Physician’s, the patient and family.
  • Focus the plan of care on the patient’s age and lifestyle to determine measurable long- and short-term goals and a defined D/C plan.
  • Discuss the proposed intervention, its material risks and expected benefits, and any reasonable alternatives with patient and family members.
  • Modifies treatment plan to coincide with patient’s goals and the overall plan.
  • Confer with the patient, medical practitioners, or appropriate others to plan, implement, or assess the intervention program.
  • Evaluate the effects of treatment at various stages and adjust treatments to achieve maximum benefit.
  • Evaluates patient outcomes based on presenting diagnosis and pertinent information regarding the patient.
  • Complete patient care documentation recording the prognosis, treatment, response, and progress in patient's chart.
  • Update medical records according to prescribed departmental standards.
  • Complete appropriate data bases, problem lists, treatment goals and individualized plan of care.
  • Completes patient charges per department guidelines.
  • Assists in precepting, orienting and teaching students (OT students, hospital staff), precepts new employees during orientation.
  • Trains and educates patients, family members and care-givers in therapy principles, methods and techniques in order to achieve intended outcomes.
  • Discharge patient from physical therapy when goals or projected outcomes have been attained and provide for appropriate follow-up care or referrals.
  • Direct, supervise, assess, and communicate with supportive personnel.
  • Assist in overseeing the activities of the staff which includes but is not limited to sourcing, hiring, and training new team members, creating and managing schedules, evaluating performance, and providing coaching to engage and motivate team members, identifying and applying career advancement opportunities for team members, and helping to resolve employee issues and disputes.
