About The Position
Signature Healthcare at Home is seeking a compassionate and skilled Physical Therapist (PT) to join our Home Health team on a PRN/hourly basis in beautiful Lincoln City, OR. This role offers flexibility and the opportunity to make a meaningful impact by providing patient-centered care in the comfort of patients’ homes along the scenic Oregon Coast. The ideal candidate is independent, patient-focused, and passionate about delivering high-quality rehabilitative care to diverse populations.
Requirements
- Graduate of an accredited Physical Therapy program
- Current Oregon Physical Therapist license (or ability to obtain)
- Minimum of 1 year of experience preferred
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal skills
- Reliable transportation, valid driver’s license, and auto insurance
- Ability to work independently and manage a flexible PRN schedule

Responsibilities
- Conduct comprehensive physical therapy evaluations in the home setting
- Develop and implement individualized plans of care based on patient needs and physician orders
- Provide hands-on therapy to improve mobility, strength, balance, and functional independence
- Educate patients and caregivers on treatment plans, exercises, and safety strategies
- Accurately document visits, progress, and outcomes in the EMR system
- Collaborate with interdisciplinary team members including nurses, OT, SLP, and physicians
- Monitor patient progress and adjust treatment plans as needed
- Ensure compliance with all regulatory, ethical, and company standards
