About The Position

This is a Full-Time Outpatient Physical Therapy position that will also involve occasional coverage in Acute Care. The role involves evaluating, planning, directing, and administering Physical Therapy programs to restore function, prevent disability, and help patients achieve maximum independence. The position requires providing direct therapy services, gathering and interpreting patient data, documenting evaluations and progress, and communicating with physicians, referral sources, and clinical staff.

Requirements

  • Bachelor's degree in Physical Therapy program required.
  • NC license for Physical Therapy required.
  • BLS required per policy guidelines.
  • Hearing (corrected) adequate for oral/aural communication.
  • Vision (corrected) adequate for reading.
  • Intelligible speech and adequate language/cognitive skills to perform job duties.
  • Sitting, standing, and walking required throughout the day.
  • Able to demonstrate any appropriate exercise and activities to patients/caregivers.

Responsibilities

  • Evaluates patients, selects and administers assessment techniques, and synthesizes information to establish appropriate patient care plans.
  • Provides direct therapy services to patients, including occasional coverage in Acute Care 7 days a week, including holidays.
  • Gathers and interprets appropriate data from the medical record, physician reports, patient, and/or family.
  • Documents evaluations, tests, progress notes, daily treatment notes, attendance records, patient/family education, outcomes, discharge notes, and charges.
  • Communicates patient's evaluation and subsequent progress to physician, referral sources, and other clinical staff.
  • Coordinates patient's schedule and treatment programs, and discharge planning.
  • Demonstrates appropriate exercises and activities to patients/caregivers.
