About The Position

This is a Full-Time Outpatient Physical Therapist position that will also have Acute Care responsibilities on occasion. The role involves evaluating, planning, directing, and administering programs of Physical Therapy (PT) in an inpatient acute level setting, upon referral by a physician. The goal is to restore function, prevent disability, and help patients reach their maximum level of independence.

Requirements

Bachelor's degree in Physical Therapy program required.
NC license for Physical Therapy required.
BLS required per policy guidelines.

Responsibilities

Evaluates patients, selects and administers assessment techniques, and synthesizes information to establish appropriate patient care plans, including necessary modalities, procedures, exercise programs, therapeutic groups, and patient education, considering the patient's diagnosis, condition, age, and discharge needs.
Provides direct therapy services to patients admitted to a level of care requiring service provision 7 days a week, including occasional holidays in Acute Care.
Gathers and interprets appropriate data from the medical record, physician reports, patient, and/or family.
Documents evaluations, tests, progress notes, daily treatment notes, attendance records, patient/family education, outcomes, discharge notes, and charges as needed.
Communicates patient's evaluation and subsequent progress to physician, referral sources, and other clinical staff.
Coordinates patient's schedule and treatment programs, and discharge planning.
