About The Position

Choice Matters Home Healthcare is a locally owned, Medicare-certified home health agency serving Northeast Florida for over 18 years. They are seeking a Physical Therapist to help patients regain mobility, independence, and confidence. The agency emphasizes a supportive work environment, flexible scheduling, and the use of AI-assisted documentation tools and the WellSky EMR to reduce after-hours paperwork and allow clinicians to focus on patient care.

Requirements

• Current Florida Physical Therapist (PT) license
• Valid driver’s license and reliable transportation
• Excellent communication and organizational skills

Nice To Haves

• Home health experience preferred, but not required
• Experience with WellSky or another home health EMR is a plus

Responsibilities

• Perform comprehensive physical therapy evaluations and develop individualized plans of care
• Evaluate and treat patients to improve mobility, strength, balance, endurance, and functional independence
• Develop evidence-based treatment plans and modify them based on patient progress
• Educate patients and caregivers on mobility, fall prevention, home exercise programs, and safe functional movement
• Collaborate with physicians, nurses, therapists, and other members of the interdisciplinary care team
• Complete accurate and timely documentation while maintaining Medicare and agency standards
