About The Position
Sage Group HealthCare Staffing is seeking a Physical Therapist (PT) for a Home Health travel/contract position in Kettering, Ohio. This is a full-time, 13-week contract role with a guaranteed 40-hour work week, Monday through Friday, 8 AM to 5 PM. The position involves providing skilled physical therapy services to patients in their homes, observing and recording patient progress, treating patients to relieve pain and restore function, and counseling patients and families on managing their condition and using equipment. The role also includes supervising PTAs and nurse assistants, completing clinical documentation, and meeting productivity standards.
Requirements
- Current Physical Therapist experience: 1- 2 years - Required
- Bachelor of Science in Physical Therapy or Similar - Required
- Current State License as Physical Therapist or Compact if applicable - Required
- BLS Certification from AHA or American Red Cross - Required
Nice To Haves
- Recent/Current Home Health PT experience of at least 1 year: Preferred
- HCHB Charting experience - Preferred
Responsibilities
- Provide skilled physical therapy services to patients in their homes as ordered by the attending physician.
- Observe, record, and report patient reactions, changes, and progress to IDT team.
- Treats patient to relieve pain, develop or restore function, and maintain maximum performance using physical means, such as, exercise, massage, heat, water, etc.
- Counsel patient and family on the disease/injury and how to manage in all phases of physical therapy.
- Instruct patient and family members on proper use of equipment such as wheelchairs, braces, walkers, crutches, canes, and other prosthetic/orthotic devices.
- Oversee and supervise therapy services of patient provided by PTAs and when appropriate, nurse assistants.
- Communicate with the PTAs and nurse aides on supervisory visit results.
- Completes all clinical documentation following agency protocol and Medicare/Federal guidelines.
- Must meet 30 productivity points per week.
