About The Position
Navitas Healthcare, LLC is seeking a licensed Physical Therapist to provide in-home therapy services in Hood River, OR, and surrounding areas. This role is ideal for skilled PTs interested in patient-focused, independent home health care with flexible scheduling.
Requirements
- Active Oregon PT license.
- 1 year of skilled home health experience required; first-timers accepted.
- BCLS certification.
- Reliable transportation for home visits.
Nice To Haves
- Oasis and EPIC experience preferred.
Responsibilities
- Provide skilled PT services in patients’ homes (up to 6 visits per day).
- Develop and implement individualized care plans to improve functional independence.
- Conduct patient assessments, monitor progress, and adjust therapy interventions as needed.
- Document all patient visits using EPIC/Oasis software.
- Ensure patient safety and adherence to home health regulations.
- Coordinate care with nurses, physicians, and other multidisciplinary team members.
