Physical Therapist Case Manager
About The Position
The Physical Therapist Case Manager provides physical therapy services to clients in their homes and communities. Services may include screening, evaluation, direct service, consultation, and instruction, in accordance with American Physical Therapy Association standards of practice. The role includes coordinating home care needs with clients, families, physicians, other providers, and pay sources to promote client independence and well-being. The case manager develops, implements, coordinates, and revises individualized plans of care, and carries out treatment activities aimed at supporting independent living for clients and families.
Requirements
- Bachelor's, Master's, and/or Doctorate degree in Physical Therapy from an accredited institution.
- Current Physical Therapist license in the State of Maine.
- Current CPR card.
- Reliable car with insurance.
Nice To Haves
- One year of Physical Therapy experience preferred.
- Home health experience preferred.
Responsibilities
- Provide physical therapy services, including screening, evaluation, direct service, consultation, and instruction, according to American Physical Therapy Association standards of practice.
- Coordinate client home care needs with clients, families, physicians, other providers, and pay sources.
- Develop, implement, coordinate, and revise plans of care with goals appropriate to each client's age, condition, and needs.
