About The Position

The VNA is seeking a full-time Physical Therapist Assistant (PTA) to work with our Indian River County Home Health division. Under the order of the physician, and under the direction of a registered physical therapist, provides skilled physical therapy interventions and health teaching to patients and their significant other/caregiver in the home setting.

Requirements

  • Current PTA license in the State of Florida
  • Minimum of 1 year experience as a PTA

Responsibilities

  • Responsibilities are directed by a registered physical therapist.
  • Administer treatments as prescribed by the physician.
  • Observing, recording activities and findings in the clinical record and reporting to the physical therapist the patient's reaction to treatment and any changes in the patient's condition.
  • Counsel and teach home program to patient and significant other/caregiver to accomplish goals.
  • Follow care plan established by a Physical Therapist which addresses problems requiring intervention.
  • Performs discharge planning in conjunction with the patient /significant other/caregiver and Physical Therapist.
  • Collaborate with scheduler on a regular basis for coordination of patient care/schedules.
  • Maintain clinical EMR and paper records according to agency, state and federal guidelines.
  • Attends case conferences/team meetings as scheduled.
  • Attend skills-fair and other mandatory educational programs.
